Complete Your Intake Form
To take part in any session, class, retreat, or program at the Network, we ask for a completed intake form. It is how we come to know you well enough to serve you well — your story, your health, your household, and what you are hoping to change. Your intake lives in your member portal, beside the classes you have registered for.
Sign in with the email address you used at checkout — your purchases and class registrations are already attached to it.
If anything above is not right, write to us through the contact page and quote the email address you used at checkout — we will find your payment in Square.
